How to Apply

Are you interested in getting a grant from Gift of Surrogacy Foundation?

Below are our eligibility requirements:

Surrogacy must be medically necessary (some medical conditions are listed below)
Citizen or legal, permanent resident of the United States (U.S.)
Resident of the State of Georgia
Embryos must be created (and frozen) by the time of application submission
A minimum of three euploid (genetically normal) embryos must be created
Surrogate agency and intended surrogate must reside in the U.S.
Frozen embryos must reside in the U.S
Additional eligibility requirements provided with the application

Significant uterine anomaly

Absence of uterus (congenital, acquired)

Medical contraindications for pregnancy:

Pulmonary hypertension
Turner’s Syndrome/Turner’s Mosaic with compromised cardiac function
Cardiac diagnosis that increases risk of comorbidity during pregnancy
Prior poor obstetric history
1. Placenta accreta
2. Uncontrolled diabetes mellitus
3. Uncontrolled hypertension
Cancer diagnosis

Uterine factor

Recurrent implantation failure (definition variable) - failed transfer of three good quality embryos vs three euploid embryos

Prior IVF with embryos in storage with subsequent maternal death

Recurrent pregnancy loss

Definition by ASRM: two or more failed clinical pregnancies

Advanced maternal age

Case by case consideration (likely age > 55 years)

Biologic Inability to

have a child

1. Single people

2.Same-sex male couples
